Wait time for scheduling an appointment can vary from a week up to 30 days. Appointments are typically offered 30 days in advance (if we contact you to schedule, the available appointment is likely within 30 days).

Please only submit the form one time. Your submission is time stamped and keeps your place on the wait list. Multiple submissions will have the most RECENT submission date next to your name and will make the wait time LONGER.
